On October 2nd, the Nobel Prize in Medicine recognized pioneering research on mRNA. Pivotal in the response to Covid-19, this innovation is now being explored to combat other diseases, sometimes far older, such as tuberculosis (TB).
Stating that research and development (R&D) of new medical innovations against TB is a major issue should be evident. As the second leading cause of death due to infectious diseases worldwide, it is estimated that over 10 million people developed TB in 2021. Current diagnostics and treatments are neither fast enough, nor sufficiently tailored, nor effective enough to eradicate the disease, not to mention their cost. The BCG vaccine – the only available option – was discovered 100 years ago, and its effectiveness is insufficient. Consequently, tuberculosis continues to cost thousands of lives every year, with the most impoverished populations paying the heaviest price.
We observe a tangible disinterest in a disease that primarily affects the most vulnerable countries in the South, and whose market is not considered lucrative enough by pharmaceutical industries. Worse, research against this disease significantly benefits the fight against other more profitable illnesses, neglecting the communities affected by tuberculosis. Despite this, we have never been closer to the goal. With 16 vaccine candidates currently undergoing clinical research globally, researchers’ remarkable efforts have led to significant progress in recent decades. These advancements are not just an opportunity but a necessity if we want to face the new challenge posed by the increasing antimicrobial resistance worldwide.
Despite increased mobilization in recent years, the international community struggles to breathe new life into this historical battle. While the Political Declaration on Tuberculosis recently adopted by the United Nations in September marks some progress, speeches and good intentions alone will not suffice to overcome this scourge. At the halfway point of the Sustainable Development Goals, which aim to end tuberculosis by 2030, it is time to translate these promises into concrete actions at all levels, starting with more investments and tangible guarantees to ensure equitable access to future medical innovations in this field.
With a historical involvement in the fight against tuberculosis and a leading role in R&D, France is an essential actor. In November, Paris will host the Union World Conference on Lung Health, a major opportunity to turn words into actions. That’s why we, actors from the research sector and civil society worldwide, call on France to seize this opportunity. The international community has set a goal to allocate $5 billion annually for R&D against TB by 2030. To live up to its responsibilities and mobilize its partners, France must set an example and contribute its fair share by investing 100 million euros per year. Known since ancient times, it is more than time to make TB history.
